One form per family. List every child: name, age, grade, and teacher for Silver Rail kids, and age for siblings.
I am the parent or legal guardian of every child named on this form. The Little Gym of Bend provides this childcare under its own rules, staff, and insurance; its staff supervise the children. The Silver Rail Children's Fund arranges and pays for the childcare and checks families in; it does not supervise children. I understand active play and gym equipment carry some risk of injury, and I accept that risk for my children. To the fullest extent the law allows, I release the Silver Rail Children's Fund and its directors, officers, and volunteers from claims arising from my children's participation, except for gross negligence or intentional harm. I authorize emergency medical care for my children if I cannot be reached. Any child under 3 will have an adult with them the whole time. I will pick up my children by 2:30 pm. The fund may not photograph or name my children beyond what I chose on the form.
